"Test, Don't Guess" Outreach Program 2025 Report

CDF’s “Test Don’t Guess” outreach program has been providing critical diabetes management supplies and resources to low-income and homeless patients in Portland, Oregon since 2017. In partnership with local, free health clinic, Wallace Medical Concern, diabetes supply kits are distributed to patients who would otherwise be unable to afford the supplies that allow them to properly manage their diabetes.

In 2025, thanks to funding from generous partners, including the National Basketball Players Association, CDF was able to continue our efforts in this space by providing “Test, Don’t Guess” supply kits to 60 patients in need during this calendar year, in addition to 50 refill kits, ensuring that patients already utilizing the program received supplies to restock their kits. To date, we have donated a total of over 500 supply kits to patients in need.

Chris and our team are grateful for Wallace Medical Concern’s assistance in ensuring these critical supplies are distributed to patients in need. We are looking forward to another year of partnership in serving those in need in our local communities.
